I acknowledge that by purchasing this membership I will comply with the rules and regulations of MDM Fitness Studio. I understand that my membership is for one (1) year. A $50 Cancellation fee will be charged for early cancellation. After completing the year commitment I will be automatically enrolled into another year commitment unless I give notice of intent of canceling before the yearly commitment ends. Membership dues are due every month for twelve months. A $35 initiation fee is billed after signing up and a $35 annual fee is billed every January 1st. All payments are made by automatic debits from bank or credit card. I also understand that my membership is non-transferable and this agreement is binding.
